Job summary

Jackson Therapy Partners is seeking a traveling Physical Therapist to help patients recover from injuries and manage chronic conditions while exploring life in new locations.

As a travel PT, you will provide home health PT services across Kitsap County with primary focus in North Kitsap and occasional county-wide coverage. APTA-approved PT degree and active state license are required, with OASIS experience and WellSky/Oasis proficiency valued.

Qualifications

• PT degree (Bachelor, Master, or Doctoral) from APTA-approved program.
• Active State License to start the assignment.
• BLS certification may be required from AHA or ARC.
• Home health experience.
• OASIS experience including SOC, ROC, Recert, Discharge, and PT evaluation documentation.

Responsibilities

• Deliver physical therapy services in home health across Kitsap County with primary focus in North Kitsap.
• Coordinate with WellSky & Oasis documentation for SOC, ROC, Recert, and discharge.
