Duties:

• Screen and evaluate patients in short-term and long-term rehab and outpatient settings
• Use standardized assessments and evidence-based practices to support interventions
• Provide rehabilitative, skilled, and medically necessary treatments
• Administer interventions based on individual patient needs
• Assess patient progress and adjust treatments accordingly, documenting per regulations
• Offer consultation and counseling to patients, families, and caregivers
• Communicate effectively with the clinical team regarding patient progress and barriers
• Provide comprehensive discharge summaries as required

Qualifications:

Licensed Physical Therapist. Open to PTs at all experience levels.
