Duties:

• Screen and evaluate patients in short-term and long-term rehab and outpatient settings
• Use standardized assessments and evidence-based practices
• Provide skilled, rehabilitative, and medically necessary treatments
• Administer interventions based on individual patient needs
• Assess patient progress and adjust treatments accordingly
• Consult and counsel patients, families, caregivers, and other providers
• Communicate effectively with the clinical team regarding patient progress
• Prepare comprehensive discharge summaries

Licensed Physical Therapist
