Job summary

Centra Healthcare Solutions, Inc. seeks an experienced or newly graduated Physical Therapist to join a skilled SNF/LTC team in Texas. This direct hire position offers a day schedule with professional growth opportunities and a commitment to patient-centered care.

You will develop plans of care, provide skilled PT interventions per physician orders, assess patient needs, and collaborate with the care team to meet goals. Current PT license and CPR certification are required.

Qualifications

• Must have graduated from an accredited PT program.
• Current state PT licensure or active license application in progress.
• Current CPR certification.
• Strong ability to develop and implement plans of care for physical therapy patients.

Responsibilities

• Develop plans of care for each physical therapy patient.
• Provide skilled PT services in accordance with physician orders.
• Assess patient needs, plan, evaluate, and modify care to meet goals.
• Collaborate with all disciplines to plan and evaluate team goals for each patient.

Requirements

• Must have graduated from an accredited school.
• Current state license as a Registered Physical Therapist / PT or proactively in the process of the application process for current state licensure as a Registered Physical Therapist / PT.
• Current CPR certification.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services / interventions in accordance with physician orders.
• Assesses patient needs, plans for, evaluates and modifies care to meet goals of physical therapy interventions.
• Collaborates with all disciplines to plan and evaluate team goals for each patient.
