Job summary

Centra Healthcare is seeking a licensed Physical Therapist (PT) for a SNF/LTC setting in Clancy, MT. This direct-hire role features a days schedule and patient-focused therapy in a collaborative team environment.

You will develop care plans for each patient, deliver skilled PT interventions per physician orders, assess needs, modify care as goals evolve, and collaborate with other disciplines to achieve optimal outcomes.

Qualifications

• Must have graduated from an accredited school.
• Current state license as a Registered Physical Therapist / PT or in the process of licensure.
• Current CPR certification.

Responsibilities

• Provide skilled physical therapy services / interventions in accordance with physician orders.
• Develop a plan of care for each physical therapy patient.
• Assess patient needs, plan for, evaluate and modify care to meet goals of physical therapy interventions.

• This is a direct hire / 'perm' position.
• Days schedule.
• This role assumes responsibility and accountability for incorporating the vision, values, mission and critical goals of the organization into their job performance.

• Current state license as a Registered Physical Therapist / PT or proactively in the process of the application process for current state licensure as a Registered Physical Therapist / PT.
• Current CPR certification.
