Job summary

The PT will be responsible for developing care plans, delivering skilled PT services according to physician orders, and collaborating with the care team to achieve patient goals. Qualifications

• Graduated from an accredited school.
• Current state license as a Registered Physical Therapist (PT) or actively pursuing licensure.
• Current CPR certification.

• Assess patient needs, plan, evaluate and modify care to meet PT goals.
• Collaborate with all disciplines to plan and evaluate team goals for each patient.
