Job summary

BJC Home Care seeks a licensed Physical Therapist to provide direct patient care in the Sullivan, MO home care setting. You will assess, treat, and educate patients and families, coordinating with physicians to ensure integrated care.

The role includes re-evaluation and adjustment of treatment plans as patients progress. The position requires a Bachelor's Degree in Physical Therapy, a valid driver's license, and state licensure.

Qualifications

• Bachelor's degree in Physical Therapy is required.
• Valid driver's license required.
• Licensed Physical Therapist in the state.

Responsibilities

• Instructs and assists patient to perform various physical activities and use of assistive devices.
• Instructs patient and family in treatment procedures.
• Re-evaluates patient and adapts treatment program for maximum benefit.
• Confers with physician to integrate therapy with overall patient care.
• Evaluates and adjusts prosthetic/orthotic devices as needed.

offers patients and their families a complete range of home care services, including skilled nursing services, adult and pediatric hospice and supportive care, rehabilitation therapy, home infusion therapy, infusion treatment rooms, home medical equipment and high-tech respiratory care. Specialty home care programs also are available, including adult and pediatric asthma, cardiac, diabetes, orthopedic and wound care programs.

The Sullivan Intermittent Home Care Department of BJC Home Care Services provides home visits to patients in Sullivan, Missouri and several nearby counties, with 24 hour on-call home care nursing supervision. Our JCAHO accredited, multi-disciplinary approach combines leading edge technology with a firm belief in the powerful recuperative advantages of receiving home care.

Preferred QualificationsRole Purpose

Provides direct patient care activities to home care or home health patients according to physician orders in the home environment including assessment, evaluation, treatment planning, implementation and instruction/supervision of physical therapy practice.

Responsibilities

• Instructs and assists patient to perform various physical activities and in use of assistive and supportive devices such as crutches, canes, and prostheses.
• Instructs patient and family in treatment procedures.
• Re-evaluates patient and adapts treatment program accordingly to achieve maximum benefit.
• Confers with physician and other practitioners to obtain additional information, suggest revisions in treatment plan and to integrate physical therapy treatment with other aspects of patients‘ care.
• Evaluates, fits and adjusts prosthetic and orthotic devices and recommends modifications to Orthotist.
