Job summary

BJC Home Care in Sullivan, MO is seeking a full-time Physical Therapist to provide direct patient care in the home setting, following physician orders and coordinating with a multidisciplinary team.

The role requires guiding patients through exercises, teaching families, re-evaluating progress, and coordinating with physicians to integrate therapy into overall care. A valid PT license and driver’s license are required; there is no on-call.

Qualifications

• Bachelor's Degree in Physical Therapy required.
• No experience required.
• Valid Driver's License and Physical Therapist License required.

Responsibilities

• Instructs and assists patient to perform various physical activities and in use of assistive devices.
• Instructs patient and family in treatment procedures.
• Re-evaluates patient and adapts treatment program to achieve maximum benefit.
• Confers with physician and other practitioners to obtain additional information and integrate treatment with other aspects of patient care.
• Evaluates, fits and adjusts prosthetic and orthotic devices and recommends modifications.

BJC Home Care offers patients and their families a complete range of home care services, including skilled nursing services, adult and pediatric hospice and supportive care, rehabilitation therapy, home infusion therapy, infusion treatment rooms, home medical equipment and high-tech respiratory care. Specialty home care programs also are available, including adult and pediatric asthma, cardiac, diabetes, orthopedic and wound care programs.

The Sullivan Intermittent Home Care Department of BJC Home Care Services provides home visits to patients in Sullivan, Missouri and several nearby counties, with 24 hour on-call home care nursing supervision. Role Purpose

Provides direct patient care activities to home care or home health patients according to physician orders in the home environment including assessment, evaluation, treatment planning, implementation and instruction/supervision of physical therapy practice.

Responsibilities

• Instructs and assists patient to perform various physical activities and in use of assistive and supportive devices such as crutches, canes, and prostheses.
• Instructs patient and family in treatment procedures.
• Re-evaluates patient and adapts treatment program accordingly to achieve maximum benefit.
• Confers with physician and other practitioners to obtain additional information, suggest revisions in treatment plan and to integrate physical therapy treatment with other aspects of patients‘ care.
• Evaluates, fits and adjusts prosthetic and orthotic devices and recommends modifications to Orthotist.
