Continuum Rehab Group is seeking a Physical Therapist to join our in-house therapy team serving Skilled Nursing/Long Term Care Facilities. You'll provide evidence-based rehabilitation to residents and collaborate with a multidisciplinary team to optimize recovery and quality of life.

Experience/Qualifications

Some Duties For This Position Include

• Provides skilled therapy services to patients who need rehabilitation services.
• Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to physical disorders.
• Supervise Physical Therapy Assistants, temporary licensees, students, and support personnel in accordance with state licensure requirements and professional standards.
