Job summary

Liberty Health Elizabethtown, North Carolina seeks a Physical Therapist (PT) to join its rehab center and coordinate patient care within a multidisciplinary team.

Responsibilities include developing individualized treatment plans and delivering in-house therapy with local management support. Eligibility requires graduation from an accredited therapy program and state licensure; prior SNF rehab experience is preferred.

Qualifications

• Education: Must be a graduate of an accredited Therapy program.
• Certificate / LICENSE: Must be licensed in the state of the SNF you will be working.
• EXPERIENCE: Previous rehab SNF experience is preferred.

Responsibilities

• Work with a multidisciplinary team to insure a wonderful rehab experience for patients and their families by being responsive and individualizing treatment plans.
• In-house therapy with local management support.
