Job summary

Liberty Health in Winston-Salem, North Carolina, is seeking a Physical Therapist (PT) for Oak Forest Health & Rehab Center. You will work with a multidisciplinary team to provide a rehab experience that is responsive and individualized for patients and their families.

In-house therapy with local management support. Must be licensed in North Carolina and graduate from an accredited therapy program; prior SNF rehab experience is preferred.

Qualifications

• Must be a graduate of an accredited Therapy program.
• CERTIFICATE / LICENSE: Must be licensed in the state of the SNF you will be working.
• EXPERIENCE: Previous rehab SNF experience is preferred.

Responsibilities

• Work with a multidisciplinary team to insure a wonderful rehab experience for patients and their families by being responsive and individualizing treatment plans.
• Be able to work in a supportive rehab environment with progressive treatment modalities to address a variety of diseases and impairments.
• In-house therapy with local management support.
