Job summary

Enhance Rehabilitation is seeking qualified Physical Therapists to join our team in a Skilled Nursing Facility setting. You will evaluate and treat patients following an individualized plan of care and collaborate with facility staff to ensure excellent patient outcomes.

As part of the team, you will supervise Physical Therapist Assistants, re-evaluate progress, revise plans of care, and complete timely documentation including evaluations, re-evaluations, discharges and daily progress notes.

Qualifications

• Licensed physical therapist in good standing.
• Experience in SNF or geriatric rehab preferred.
• Strong communication and patient education skills.

Responsibilities

• Evaluating new patients, developing plans of care and setting measurable, functional goals.
• Treating patients, addressing concerns and educating them on progress and outcomes.
• Complete timely and accurate documentation including evaluations, re-evaluations, discharges and daily progress notes.
• Supervising Physical Therapist Assistants and coordinating with facility staff for optimal patient care.
