Job summary

Enhance Rehabilitation is seeking qualified Physical Therapists to join our team in SNF settings. You will evaluate and treat patients following an individualized plan of care and work closely with facility staff to ensure high-quality patient outcomes.

You will supervise PT Assistants, re-evaluate progress, adjust plans of care, and complete timely documentation. Exceptional patient service and clear communication with patients and families are essential to success in this role.

Qualifications

• Evaluating new patients and developing plans of care.
• Setting measurable, functional goals.
• Providing patient education on injuries, progress, and outcomes.

Responsibilities

• Providing excellent care for our patients
• Providing excellent customer service to patients and family members
• Collaborating with facility staff to ensure excellent patient care
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Supervising Physical Therapist Assistants
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
