Job summary

Enhance Rehabilitation is seeking qualified Physical Therapists to join our dynamic team in SNF settings. You will evaluate and treat patients following an individualized plan of care, establishing measurable goals and tracking progress.

You will collaborate with facility staff, educate patients and families about injury and outcomes, and supervise Physical Therapist Assistants while ensuring timely documentation of evaluations and progress notes.

Responsibilities

• Evaluating new patients and developing plans of care.
• Setting measurable, functional goals for patient progress.
• Treating patients and educating them on outcomes.
• Collaborating with facility staff to maintain care quality.
• Supervising Physical Therapist Assistants.
• Re-evaluating progress and revising plans of care.
• Completing timely documentation including evaluations and daily progress notes.
• Providing excellent patient care and customer service.
