Job summary

Enhance Rehabilitation is seeking a Physical Therapist to join our team in a SNF setting. You will evaluate and treat patients following an individualized plan of care, collaborating with facility staff to ensure excellent patient outcomes.

The role focuses on delivering high-quality care, educating patients and families, supervising Physical Therapist Assistants, and maintaining thorough documentation of evaluations, progress notes, and discharges.

Responsibilities

• Providing excellent care for our patients
• Providing excellent customer service to our patients and family members
• Collaborate with facility staff to ensure excellent care for you patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Supervising Physical Therapist Assistants
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
