Job summary

Evolve Therapy Services is seeking a compassionate Physical Therapist to join our in-house skilled nursing facility partner in Sutherland, Nebraska. This PRN role focuses on planning, delivering and coordinating PT services to residents in a high-quality care setting.

You will evaluate residents, develop treatment plans, document per CMS in NetHealth, and collaborate with the interdisciplinary team. An active PT license in Nebraska and ongoing professional development are required.

Qualifications

• Active Physical Therapy license in Nebraska or other applicable states.
• Bachelor's degree in Physical Therapy from an accredited program.
• Ongoing continuing education to maintain licensure and certification.
• Maintain licensure and certification through ongoing professional development.

Responsibilities

• Provide PT services to residents in accordance with standards and facility procedures.
• Document and bill PT services per CMS requirements.
• Evaluate residents to determine needs and goals.
• Develop short- and long-term treatment plans and implement them.
• Communicate findings and plans to residents, families, and the care team.
• Complete compliant documentation in NetHealth EMR.
