Kadima Health and Nursing at Luzerne is seeking a Physical Therapist to join our team. You will evaluate, plan, and administer treatments for residents to relieve pain, restore function, and promote healing, within a SNF/long-term care setting.

The role requires a PA license in good standing; experience in long-term care and proficiency with documentation systems are preferred. Excellent communication with residents, families, and caregivers is essential.

Qualificações

• SNF/Long term care experience preferred.
• Knowledge of current treatment practices.
• Experience with Net Health or similar EHR is a plus.
• Ability to manage patients with different personalities.

Responsabilidades

• Evaluate, plan, and administer treatment to residents to relieve pain and improve function.
• Ensure all treatment aligns with an established plan of care.
• Provide clinical support and educate patients, families, and caregivers.
• Record and monitor motor function and muscle performance.
• Adjust treatments based on patient responses during sessions.
• Complete documentation per company policy and regulations.
