Rock Creek Care Center is a five-star Medicare-certified, short-term rehabilitation and skilled nursing home located in Auburn, California. The role involves providing physical therapy interventions to help patients achieve their goals and improve their physical functioning. Responsibilities include utilizing therapeutic exercises, manual therapy techniques, modalities, and equipment to enhance strength, mobility, balance, and coordination. Educate patients on exercises to be performed at home, assess and document patients' progress, and adjust treatment plans accordingly.
