Job summary

Apple Rehab is seeking a Physical Therapist in our Berlin, CT location. The PT will evaluate and treat patients, develop individualized rehabilitation plans, and collaborate with physicians to maximize patient outcomes.

Responsibilities include administering therapy, documenting progress, and guiding patients and families on home programs. Pride in quality care, competitive benefits, and a family-like work environment are emphasized.

Qualifications

• Graduate of an accredited PT program required.
• Current PT license in good standing with the state licensing board.
• Federal-, State- and GHR-required credentials current and on file.
• Cognitive and clinical skills necessary for physical therapy practice.

Responsibilities

• Plans and administers physical therapy treatment to restore function.
• Evaluates patient condition and develops customized rehabilitation plan.
• Documents treatment progress and communicates with physician.
• Instructs patients and families on home treatment and safety.
• Supervises assistants and aides as assigned.
• Ensures adherence to HIPAA and safety protocols.

Education
