Qualifications

• Current Oregon Physical Therapist license.
• Graduate of an accredited CAPTE‑approved Physical Therapy program.
• Minimum 2 years of Physical Therapist experience; home health experience preferred.
• Current CPR certification.
• Excellent communication, organizational, and documentation skills.
• Valid driver's license, reliable transportation, and current auto insurance.

Responsibilities

• Develop and implement individualized treatment plans.
• Perform patient assessments, including mobility, gait, strength, and ROM evaluations.
• Provide physical therapy services per physician-directed care plans.
• Educate patients, families, and caregivers on goals, exercises, and safety.
• Monitor patient progress and maintain accurate clinical documentation.
• Collaborate with physicians and interdisciplinary healthcare teams.
• Participate in care conferences and discharge planning.
• Supervise Physical Therapy Assistants and home health aides as required.
• Identify additional patient care needs and coordinate appropriate referrals.
