Job summary

Qualifications

• Active Physical Therapist license in the state of practice.
• Ability to perform essential job functions with accommodations as needed.

Responsibilities

• Evaluates patients and develops individualized treatment plans.
• Provides direct therapy services and trains patients and caregivers.
• Maintains documentation including evaluation results and progress notes.
• Communicates with the patient, family, and care team about status and goals.
• Oversees PT assistants and students to ensure safe patient care.
