Job summary

The Physical Therapist at Sandstone PA provides rehabilitative therapy to residents in a skilled nursing facility, following regulatory and clinical standards. This role aims to improve cognitive/ADL functioning and restore independence where possible.

Position reports to the Director of Rehabilitation Services. Pay rate is $42-$50 per hour with sign-on bonuses of $6,000 for full-time and $3,500 for part-time. Opportunities include ongoing training and a collaborative team environment.

Qualifications

• Bachelor's degree or higher in Physical Therapy or related field.
• Active state license.
• 2+ years of experience in a skilled nursing facility environment preferred.

Responsibilities

• Screen/evaluate patients with cognitive/ADL issues and develop care plans.
• Utilize standardized assessments and evidence-based practice.
• Provide rehabilitative, skilled treatment per clinical requirements.
• Administer ADL retraining, exercises, modalities and devices.
• Assess progress and update clinical records accordingly.
• Document outcomes and provide discharge summaries per policy.
• Communicate progress with supervisor and health team.
• Supervise PT assistants and students.
