Job summary

Sandstone Post Acute is seeking a compassionate Physical Therapist to join our skilled nursing team. You will evaluate, treat, and document patient progress, focusing on cognitive and ADL functioning, and work under regulatory standards to improve quality of life for residents.

Qualifications

• Bachelor's degree or higher in Physical Therapy or related field.
• Active state license.
• 2+ years of experience in a skilled nursing facility environment preferred.

Responsibilities

• Screen and evaluate patients with cognitive and/or ADL deficits and develop care plans.
• Use standardized assessments and evidence-based practice.
• Provide rehabilitative therapy per regulatory standards.
• Administer ADL retraining, therapeutic activities, exercises, and modalities.
• Document progress and adjust treatment plans in the medical record.
• Evaluate outcomes and refer as needed based on progress.
• Communicate with patients, families, and healthcare team about plans and progress.
• Prepare discharge summaries and ensure proper documentation.
