Job summary

CommonSpirit Health is seeking a dedicated Physical Therapist to provide comprehensive rehabilitation for patients with neuromuscular and orthopedic conditions. The role includes mobility training, strengthening, manual therapy, and neuromuscular re-education to restore optimal health.

Responsibilities include initial evaluations, creating goal-driven treatment plans, executing therapy activities, and thorough documentation while collaborating with the healthcare team to support discharge

Qualifications

• Bachelor's degree in Physical Therapy required.
• Licensed PT or eligible licensure in NLC states.
• BLS certification from American Heart Association.

Responsibilities

• Evaluate patients referred to physical therapy within department time standards.
• Develop treatment plans with measurable goals and timelines.
• Deliver therapy activities per plan of care to meet goals.
• Document patient evaluations, progress notes, and treatment records.
• Reassess and adjust plans based on patient function and discharge needs.
• Communicate with patients, families, staff, and physicians while maintaining confidentiality.
