Responsibilities

• Evaluates patients referred to physical therapy and performs initial evaluations.
• Creates treatment plans with measurable goals and time frames.
• Selects and implements therapy activities per plan of care to meet goals.
• Documents all patient care activities per standards of practice and payer requirements.
• Reassesses and modifies treatment plans based on patient progress.
• Communicates with patients, families, staff, and physicians regarding services.
