Job summary

Cortlandt Healthcare in Cortlandt Manor, NY is seeking a full-time Physical Therapist to join our in-house rehab team. You will interview with the people you will work with on site, supported by a 35-facility network for professional growth.

New graduates are trained with a mentor and a gradual caseload ramp, while ongoing in-house education keeps you advancing. Leadership is accessible to help plan your five-year path.

Qualifications

• Doctor of Physical Therapy or equivalent required.
• Valid New York Physical Therapist license or license eligible.
• Ability to supervise Physical Therapist Assistants as per state act.

Responsibilities

• Complete physical therapy evaluations covering strength, ROM, balance, gait, transfers, endurance and fall risk.
• Set functional, measurable goals tied to discharge planning and prior function.
• Deliver skilled interventions including therapeutic exercise, gait training, balance and neuromuscular reeducation.
• Complete weekly progress notes, recertifications and discharge documentation on time.
• Train residents, families and caregivers on safe mobility and home carryover.
• Partner with nursing on safe transfer status, fall prevention and restorative programming.
• Direct and supervise PTAs and students per state practice act.
