Job summary

Continuum Therapy Partners, LLC is seeking a licensed Physical Therapist in West Reading, PA for a full-time daytime role. The successful candidate will evaluate, develop, and implement individualized treatment plans, coordinate with other disciplines, and document patient progress.

Requirements include a Doctor of Physical Therapy, active state license with NPTE, and ideally 1 year of experience. Benefits include full package, licensure reimbursement, and PTO, with a $5k sign-on or $10k

Qualifications

• DPT degree required.
• Current, unencumbered state Physical Therapy License; must pass the NPTE.
• 1 year of previous experience as a Physical Therapist preferred.
• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.
• Educates patients and families on home exercise programs and injury prevention.
