Qualifications

• Bachelor’s degree in Physical Therapy from an accredited college or university.
• Must maintain an active, unencumbered PT license in the state of employment.
• Must possess basic computer skills and navigate service delivery and EHR software.

Responsibilities

• Return patient to optimal physical function in a SNF setting.
• Instruct patients in independent exercise programs and activity modification.
