Saber Health Group is seeking a dedicated Physical Therapist to join our skilled nursing facility team and help residents regain independence and mobility. You will assess, plan, and implement therapy programs to achieve physician and patient goals.

You will educate patients on home exercises and activity modification, ensuring safe discharge and ongoing functional maintenance. The role includes full-time shifts in North Carolina and access to growth opportunities through education programs.

Qualifications

• Bachelor’s degree or higher in Physical Therapy from an accredited college or university.
• Active, unencumbered Physical Therapy license for the state of employment.
• Basic computer skills and ability to navigate service delivery and electronic health record software.

Responsibilities

• Restore patients to optimal physical function and support therapy goals in a SNF setting.
• Educate patients to maintain function after discharge via independent exercise program and activity modification.
