Job Title: Physical Therapist
Hours: 30-40 per week; Generally M-F, 8:45 a.m. – 5:00 p.m. (flexible schedule work: Sunday - Thursday or Tuesday - Saturday); weekends if necessary
We are seeking a dedicated and compassionate full‑time Physical Therapist to join our team. This role offers an excellent opportunity to make a meaningful difference in patients' lives by helping them regain mobility, reduce pain, and improve their overall quality of life through personalized rehabilitative care.
Essential Job Duties
- Restore patient’s function, alleviate pain and prevent disabilities by planning and administering medically prescribed physical therapy.
- Provide quality care by assessing and interpreting evaluations and test results.
- Develop a treatment plan for each patient based on initial and ongoing evaluations, including short- and long‑term goals, collaboration demands of other staff members, and required patient and caregiver involvement.
- Determine physical therapy treatment plans in consultation with physicians or by prescription.
- Implement Plan of Treatment on a consistent schedule and adjust this schedule and plan according to patient progress.
- Help patient accomplish treatment plan by administering manual exercises, massage, and using equipment as necessary.
- Evaluate mobility, strength, and current level of patient function, while setting goals to improve in these areas that are scaled to patient abilities and realistically attainable.
- Evaluate and record patient progress, modify treatment plans, and try new treatments if required.
- Evaluate the use and fit of prosthetic and orthotic devices by observing, noting patient’s progress, and recommending adjustments and modifications.
- Document patient care services according to approved medical documentation and facility documentation procedures.
- Report patient progress to the patient, patient caregivers and loved ones, facility staff, and healthcare professionals involved in ongoing treatment/care of patients.
- Develop a plan for patients following completion of their Plan of Treatment with appropriate in‑home care services.
- Lead by example and exhibit professionalism at all times.
- Hold themselves accountable and encourage collaboration and flexibility within the department and interdepartmental teamwork for positive outcomes.
Requirements
Qualifications
- Education: Graduate of an accredited therapy program
- Experience: 2-3 years of related experience; or an equivalent combination of education and experience
- Licenses/Certifications: Current Physical Therapist state license; CPR certification
- Excellent verbal & written communication

- Compassionate, patient‑focused approach to care
- Previous experience in a healthcare setting, preferably with geriatric populations is a plus
