Position: Full-Time Physical Therapist, PT

Location: Center City, serving adult and geriatric clients in a home health setting.

About the Role

As a home care Physical Therapist, you will be a vital part of a multidisciplinary healthcare team, providing rehabilitative care and skilled nursing to help clients remain at home. Responsibilities include making home visits, performing diagnostic tests, developing and revising care plans, and educating clients and their families.

Responsibilities

1. Make home visits to clients in designated areas.
2. Perform diagnostic assessments such as mobility, strength, and safety evaluations.
3. Implement wound care and other therapeutic interventions.
4. Create and adjust personalized care plans.
5. Educate clients and families on rehabilitative activities.
6. Document care activities accurately using electronic devices.

Qualifications

• Current Pennsylvania PT license.
• At least one year of recent PT experience.
• Graduation from an accredited Physical Therapy program.
• Ability to work independently and manage time effectively.
• Strong interpersonal and organizational skills.
• Proficiency with computers; experience with EMR preferred.
