Job description

BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) for a Part-Time or Per Diem opportunity. The role involves home health visits to adults and seniors in Senior Living Communities throughout Cumberland and Franklin Counties.

Requirements include at least one year of clinical experience as a licensed Physical Therapist. The PT will be part of a multidisciplinary team providing rehabilitative care and skilled nursing, enabling clients to remain at home.

Responsibilities:

• Make home visits within designated geographic areas.
• Perform diagnostic tests and measurements, including mobility, strength, balance, and safety assessments.
• Implement wound care interventions using evidence-based dressings and topical agents.
• Develop and revise individualized care plans, including manual therapy, gait training, and balance exercises.
• Assess and update care plans regularly, collaborating with nursing staff.
• Educate clients and families on rehabilitative activities to promote health and independence.
• Document client observations, interventions, and evaluations accurately using electronic tablets.

Qualifications:

• Current Pennsylvania Physical Therapist (PT) license.
• Minimum of one year of recent PT experience.
• Graduation from an APTA-approved program or equivalent.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Proficiency with computers; EMR experience preferred.
