Job summary

St. Luke's is seeking a Physical Therapist to evaluate, diagnose and manage patients with physical impairments, and to maximize functional abilities.

You will design and implement individualized treatment plans and educate patients and families on prevention and wellness. You will perform comprehensive initial evaluations, tailor interventions, set goals with patients, and document progress while aligning with department standards and patient safety guidelines.

Qualifications

• Licensed physical therapist with Doctoral or professional credential in physical therapy.
• Ability to assess, diagnose, and plan treatment and evaluate patient progress.
• Strong communication with patients, families and care teams.

Responsibilities

• Performs initial evaluations and develops treatment plans.
• Provides skilled interventions tailored to patient needs.
• Collaborates with patients to set short and long-term goals.
• Maintains accurate patient records and progress notes.
• Educates patients and families on prognosis and care options.
• Supervises PTAs and students as assigned.
