Job summary

Kern River Transitional Care is seeking a compassionate, Full-Time Physical Therapist to join our rehabilitation team. The role focuses on improving mobility, independence and quality of life for residents in a supportive, interdisciplinary environment.

You will evaluate, treat, and document progress, collaborating with physicians, nurses and therapists to achieve measurable milestones and safe patient care. Competitive hourly pay and comprehensive benefits accompany this position.

Qualifications

• Graduate of an accredited physical therapy program.
• Current California Physical Therapist license required.
• CPR/BLS certification required.
• Experience in skilled nursing/geriatric rehab preferred.

Responsibilities

• Evaluate residents' mobility, strength, balance, range of motion and functional abilities.
• Develop individualized treatment plans based on resident needs and goals.
• Provide skilled PT services to improve mobility, independence, safety and function.
• Assist residents to regain walking, transfers, balance and daily activities.
