Qualifications

• Graduate of a CAPTE-accredited physical therapy program.
• Current licensure to practice physical therapy in Michigan (or eligible to obtain).
• BLS certification and continuing education to meet state requirements.

Responsibilities

• Evaluate patients and determine PT needs.
• Develop and implement individualized treatment plans.
• Set measurable goals with patients and families, in collaboration with the care team.
• Monitor progress and adjust treatment plans to optimize outcomes.
• Document patient status and treatment per guidelines and time standards.
• Coordinate discharge planning and with inter-disciplinary teams.
