Job summary

CommonSpirit Health is seeking a Physical Therapist to provide inpatient and outpatient services at St. Luke's Health - Patients Medical Center in Pasadena, TX. You will develop and update care plans, work with the patient and family, set measurable goals, and coordinate discharge planning.

You will also discharge when goals are met, maintain productivity standards, collaborate with the interdisciplinary team, document thoroughly, and participate in patient education and outcomes measurement.

Qualifications

• CPR/BLS certification.
• Degree in Physical Therapy.
• State Licensure in Physical Therapy (PT).
• Graduates eligible for licensure will be considered.

Responsibilities

• Develop and update plan of care with patient/family and set measurable short- and long-term goals.
• Follow intervention protocols; discharge when goals met or medical necessity no longer warrants intervention.
• Maintain productivity and collaborate with supervisors and peers to support department goals.
• Provide professional customer service and work with interdisciplinary team to meet patient needs in a timely manner.
• Document thoroughly per policy including charting, charge entry, and daily schedules.
