Heritage Oaks in Georgia is seeking a licensed Physical Therapist to join our Rehabilitation Services team. You will deliver skilled nursing facility rehabilitation, supervise PT Assistants, and collaborate with the Rehabilitation Director and multidisciplinary staff to optimize patient outcomes.

The ideal candidate holds a Bachelor's or Master's in Physical Therapy, an active Georgia license, and at least two years of SNF rehab experience.

Qualifications

• Bachelor’s or Master’s degree in Physical Therapy.
• Current, unencumbered Georgia Physical Therapy license.
• Two years SNF rehabilitation experience preferred.

Responsabilités

• Contributes to a positive work team by sharing information and problem-solving.
• Provides supervision of Physical Therapist Assistants as per GA board rules.
• Maintains ongoing collaboration with Rehabilitation Director and disciplines.
• Maintains up-to-date knowledge through seminars and journals.
• Delivers safe, high-quality patient care and effective case management.
