Job summary

Heritage Oaks in Georgia is seeking a licensed Physical Therapist to join our Rehabilitation Services team. You will contribute to high-quality patient care by applying current treatment knowledge within a skilled nursing facility setting and collaborating with an interdisciplinary team.

Responsibilities include supervising Physical Therapist Assistants, documenting clearly, and supporting continuous quality improvements.

Qualifications

• Bachelor’s or Master’s degree in Physical Therapy.
• Current Georgia license to practice as a Physical Therapist.
• Prefer two years experience in a skilled nursing facility rehabilitation environment, or equivalent.

Responsibilities

• Contributes to a positive work team by sharing information and problem-solving.
• Supports activities as introduced by skilled nursing facility and Rehabilitation Services.
• Demonstrates superior customer service through courtesy and follow-up.
• Provides supervision of Physical Therapist Assistants as defined by Georgia rules.
• Collaborates with the Rehabilitation Director and other disciplines for case management.
• Maintains knowledge through seminars and journals for quality improvements.
• Reports for scheduled work periods on time.
• Performs other duties to ensure the System’s success.
• Promotes the System’s image through servant leadership and open communication.
