Job Summary and Qualifications

The Physical Therapist (PT) provides high-quality, patient-centered care, managing therapeutic care for assigned patients, collaborating with physicians and care teams, and developing individualized care plans.

Responsibilities include:

• Providing evaluations and treatment within legal and ethical scope
• Assessing therapeutic/functional status and developing care plans
• Delivering therapeutic care at patient’s home, using appropriate procedures and techniques
• Instructing patients and families on therapies and device use
• Prioritizing caseload and coordinating staffing and handoffs

Qualifications:

• Basic Cardiac Life Support (BLS) within 30 days of start
• Licensed Physical Therapist in Texas
• Degree from an accredited PT program (Doctorate, Bachelors, or Masters)
• 1-2 years rehab experience preferred; new grads considered
• Availability to work weekdays and rotating weekends
