Job summary

Trilogy Health Services is seeking a Physical Therapist to provide a full range of PT services in a senior living setting. You will assess patients, develop personalized treatment plans, and apply therapeutic interventions to improve function.

Responsibilities include patient education, documentation, and collaboration with an interdisciplinary team. Travel is required as needed, with 10% or less typically anticipated.

Qualifications

• Master's degree in physical therapy.
• 0-1 years of experience.
• Current PT licensure in state.
• CPR certification preferred.

Responsibilities

• Perform comprehensive assessments to determine patient needs and rehabilitation goals.
• Develop and implement individualized treatment plans with exercises, manual therapy, and modalities.
• Educate patients and families on exercises, posture, and body mechanics.
• Maintain accurate documentation of progress, plans, and outcomes per facility policies and regulatory requirements.
• Collaborate with an interdisciplinary team to ensure comprehensive patient care.
