Concept Rehab is seeking a Physical Therapist to join our skilled nursing rehabilitation team in Johnston, Iowa. You will evaluate patients, develop personalized care plans, and deliver evidence-based PT interventions to improve mobility and independence.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Iowa Physical Therapist license or eligibility for licensure.
• Ability to work collaboratively within an interdisciplinary rehabilitation team.
• Passion for serving older adults in a Skilled Nursing Facility and post-acute setting.

Responsibilities

• Evaluate patients following physician referral and establish individualized plans of care.
• Develop functional treatment goals that promote mobility and independence.
• Deliver skilled physical therapy interventions in accordance with evidence-based practice.
• Assess gait, balance, strength, endurance, transfers, and functional mobility.
• Supervise and collaborate with Physical Therapist Assistants (PTAs).
• Communicate patient progress with physicians, nursing staff, families, and interdisciplinary team members.
• Participate in care conferences and discharge planning meetings.
• Educate patients and caregivers regarding safety, mobility, and home exercise programs.
• Maintain accurate and timely clinical documentation.
• Support quality outcomes and patient satisfaction initiatives.
