Job summary

Concept Rehab is seeking a Physical Therapist (PT) to join our Skilled Nursing Facility team in Johnston, Iowa. We offer full-time, part-time, and PRN opportunities with a supportive clinical environment and caregiver-focused care for older adults.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Iowa Physical Therapist license or eligibility for licensure.
• Strong communication and interpersonal skills.
• Ability to work collaboratively within an interdisciplinary rehabilitation team.
• Passion for serving older adults in a Skilled Nursing Facility and post-acute rehabilitation setting.

Responsibilities

• Evaluate patients following physician referral and establish individualized plans of care.
• Develop functional treatment goals that promote mobility and independence.
• Deliver skilled physical therapy interventions in accordance with evidence-based practice.
• Assess gait, balance, strength, endurance, transfers, and functional mobility.
• Supervise and collaborate with Physical Therapist Assistants (PTAs).
• Communicate patient progress with physicians, nursing staff, families, and interdisciplinary team members.
• Participate in care conferences and discharge planning meetings.
• Educate patients and caregivers regarding safety, mobility, and home exercise programs.
• Maintain accurate and timely clinical documentation.
• Support quality outcomes and patient satisfaction initiatives.

Concept Rehab is seeking a Physical Therapist (PT) to join our rehabilitation team in a Skilled Nursing Facility (SNF) in Johnston, Iowa. We have Full-Time, Part-Time, and PRN opportunities available for licensed Physical Therapists who are passionate about helping older adults regain mobility, independence, and quality of life following illness, injury, surgery, and hospitalization.
