Job summary

CommuniCare Rehab in Cincinnati is seeking a full-time Physical Therapist to evaluate and treat residents at Pleasant Ridge Healthcare Center. You will work as an in-house team member delivering direct therapy and coordinating with staff to implement care plans.

This role offers a $5,000 sign-on bonus and collaboration with a multidisciplinary rehab team across centers. A valid State PT license is required or pending, with opportunities to impact residents’ quality of life.

Qualifications

• Bachelor’s, Master’s, or Doctorate in Physical Therapy from an accredited program.
• Hold a valid or pending State Physical Therapy License.

Responsibilities

• Evaluate, review, and screen patient’s/resident’s medical charts and evaluations under physician order to develop a treatment plan.
• Implement treatment plan through direct care and educate the plan of care to all team members, including supervising PTAs.
• Attend/lead interdisciplinary meetings to relay clinical findings and make rehab recommendations; train staff and families as appropriate.
• Opportunities to work at multiple centers to serve a wide variety of patient needs.

Education

Bachelor’s, Master’s, or Doctorate Degree in Physical Therapy from an Accredited Program.
