Human Touch Home Health in Rockville, MD is seeking Occupational Therapists for full-time, part-time, and PRN home health roles across Rockville, Gaithersburg, Olney, and surrounding Montgomery County. You will perform comprehensive OT evaluations, develop care plans, and help patients improve independence with ADLs, while coordinating with physicians and the care team.

Qualifications

• Active Maryland OT license required.
• Graduate of an accredited OT program.
• Strong assessment and treatment skills.
• Excellent communication and documentation abilities.

Responsabilités

• Perform comprehensive OT evaluations.
• Develop individualized plans of care.
• Help patients improve independence with ADLs.
• Provide therapeutic interventions to improve functional ability.
• Address bathing, dressing, grooming, toileting, meal preparation and household activities.
• Assess home safety and fall risks.
• Recommend adaptive equipment and home modifications.
• Educate patients and caregivers.
• Monitor progress and update treatment plans.
• Complete reassessments and discharge planning.
• Coordinate with physicians and the interdisciplinary team.
• Complete accurate documentation using WellSky.

What We're Looking For

• Reliable transportation
• Strong assessment and treatment skills
• Excellent communication and documentation abilities
• Ability to work independently
• Home Health Experience Preferred, but Not Required
• WellSky Experience is a Plus
