Job summary

Flexible scheduling and reliable transportation required for all roles. Responsibilities include in-home visits, evaluations, reassessments, fall risk assessments, and accurate documentation communicated with the care team.

Qualifications

• Licensed Physical Therapist or Physical Therapist Assistant in Hawaii.
• 1+ year of clinical experience; home health preferred.
• Strong documentation and communication skills.
