Job summary

O’ahu Home Health Agency is seeking licensed Physical Therapists (PT) and Physical Therapist Assistants (PTA) for per diem, in-home visits across O’ahu, HI. Flexible schedule, travel to patient homes, and the chance to build a varied caseload.

Responsibilities include conducting evaluations, follow-ups, and fall risk assessments, documenting progress, and coordinating with the care team to keep patients safe and mobile at home. Must have Hawaii license and reliable transportation.

Qualifications

• Current Hawaii license as PT or PTA.
• 1+ year of clinical experience; home health preferred.
• Strong documentation and communication skills.

Potential to Earn $100,000–$120,000/Year

Pay:

• PT: $80 per visit
• PTA: $55–$60 per visit

Location: O'ahu, HI (reliable transportation required)
Schedule: Flexible / Per diem — choose your own patients
