Job summary

Sunnyvale Post Acute is seeking a PRN Weekend Physical Therapist to join our rehab team. You will conduct evaluations, develop treatment plans, and deliver evidence-based therapy to residents in a post-acute/long-term care setting, working with an interdisciplinary team to optimize mobility and independence.

Requirements include a California PT license, graduation from an accredited PT program (DPT or MSPT), and ability to work weekends.

Qualifications

• Current California Physical Therapist (PT) license.
• Graduate of an accredited PT program (DPT or MSPT).
• Previous SNF or post-acute rehab experience preferred (not required).
• Strong clinical reasoning and communication skills.
• Ability to work independently and collaboratively.

Responsibilities

• Complete PT evaluations and develop individualized treatment plans
• Provide effective, evidence‑based therapeutic interventions
• Document treatments, progress notes, and evaluations in accordance with facility and regulatory requirements
• Communicate patient progress with interdisciplinary team members
• Educate patients, families, and caregivers on therapy goals, safety, and home programs
• Ensure safe, ethical, and high‑quality care at all times

Pay Rate: $57-$67 per hour (DOE)
Schedule: PRN / Weekends

Position Summary

The PRN Physical Therapist evaluates, plans, and provides patient‑centered physical therapy services for individuals requiring rehabilitation in a post‑acute/long‑term care setting. You will collaborate closely with an interdisciplinary team to ensure the best rehabilitation outcomes for our residents.
