Job summary

Sunnyvale Post Acute in Sunnyvale, CA is seeking a PRN Weekend Physical Therapist to join our rehab team. You will evaluate and treat residents in a post-acute/long-term care setting, delivering patient-centered care with an interdisciplinary team.

Qualified candidates hold a CA PT license (DPT/MSPT preferred) and have strong clinical reasoning and communication skills. SNF or post-acute rehab experience is preferred but not required. Competitive hourly rate and flexible weekend scheduling.

Qualifications

• Licensed as a Physical Therapist in California (PT).
• Graduated from an accredited PT program (DPT or MSPT).
• SNF or post-acute rehab experience preferred.
• Strong clinical reasoning and communication skills.
• Ability to work independently and with a team.

Responsibilities

• Complete PT evaluations and develop individualized treatment plans
• Provide effective, evidence-based therapeutic interventions
• Document treatments, progress notes, and evaluations in accordance with facility and regulatory requirements

Location: Sunnyvale Post Acute – Sunnyvale, CA
Pay Rate: $57–$67 per hour (DOE)
Schedule: PRN / Weekends

We are seeking a talented PRN Weekend Physical Therapist to join our rehab department.

Position Summary

The PRN Physical Therapist evaluates, plans, and provides patient‑centered physical therapy services for individuals requiring rehabilitation in a post‑acute/long‑term care setting. You will collaborate closely with an interdisciplinary team to ensure the best rehabilitation outcomes for our residents.
