Job summary

Sunnyvale Post Acute in Sunnyvale, CA is seeking a licensed Physical Therapist to join our rehab team for PRN weekend coverage. You will evaluate residents, develop individualized treatment plans, and deliver evidence-based PT interventions in a post-acute/long-term care setting.

Collaborate with an interdisciplinary team, document progress, educate patients and families, and maintain high standards of safety and quality care.

Qualifications

• California PT license required.
• Graduate of an accredited PT program (DPT or MSPT).
• SNF or post-acute rehab experience is preferred but not required.

Responsibilities

• Complete PT evaluations and develop individualized treatment plans.
• Provide effective, evidence-based therapeutic interventions.
• Document treatments, progress notes, and evaluations in accordance with facility and regulatory requirements.
• Communicate patient progress with interdisciplinary team members.
• Educate patients, families, and caregivers on therapy goals, safety, and home programs.
• Ensure safe, ethical, and high-quality care at all times.
