Job summary

Interim HealthCare of Central Montana is seeking a Home Health Physical Therapist in Great Falls, MT. You will provide home-based PT to patients, coordinating with a home health team to deliver personalized, goal-driven care in the patient’s home.

The role emphasizes autonomy, work-life balance, and meaningful patient interactions. A MT PT license and CPR are required, with 2+ years in PT preferred, and benefits include 1:1 care, training, and competitive pay.

Qualifications

  • Graduate of an accredited Physical Therapist Program and active MT PT license.
  • Minimum of two years PT experience, ideally in home health care.
  • CPR certification required.
  • Hep B and TB immunizations required.
  • Knowledge of state/federal home health regulations; strong clinical judgement.

Responsibilities

  • Provide home-based physical therapy to patients with mobility/strength limitations.
  • Collaborate with home health team (RN, LPN, CNA, HHA, OT, SLP) to follow the plan of care.
  • Observe movements, assess progress, document changes, adjust care plan as needed.
  • Assist patient with exercises and hands-on therapy to improve mobility and reduce pain.
  • Educate patients on plan of care, goals, exercises and self-care.
  • Coach family on safety, exercises, and care responsibilities.

As a Home Health Physical Therapist, here’s a big-picture view of what you’ll do:
  • Provide home-based physical therapy to patients who have lost mobility, strength and functionality due to an illness or injury
  • Work as part of a home health team which may include an RN, LPN, CNA, HHA, OT and SLP, focused on the patient’s plan of care and goals
  • Observe movements, conduct assessments, document progress and modify care plan as needed
  • Assist patient with exercises and hands-on therapy to manage pain and increase mobility
  • Educate patients on their plan of care, established goals, exercises and self-care
  • Coach family on the plan of care, therapy goals and safely assisting with exercises
A few must-haves for Home Health Physical Therapists:
  • Graduate of an accredited Physical Therapist Program and active PT license in Montana
  • Minimum of two (2) years of physical therapy experience, ideally in home healthcare
  • CPR certification
  • Hep B and TB immunizations
  • Knowledge of state and federal home health regulations
  • Good clinical judgement, strong interpersonal skills, dexterity and compassion
